The Release.

In the event that neither myself nor an Emergency Contact can be reached I, the Owner, authorize, Your Pooches BF or the Pet Care Professional at the time of service, to obtain whatever emergency veterinary care that Your Pooches BF or the Pet Care Professional deems necessary. I will prepare with reasonable financial resources (a check left with the Pet Care Professional, or credit card on file) to incur veterinary costs for the Pets in my name.

Acknowledgment

By signing this Electronic Signature Form, I agree that my electronic signature is the legally binding equivalent to my handwritten signature. I will not, at anytime in the future, repudiate the meaning of my electronic signature or claim that my electronic signature is not legally binding.
